Policies
Payment
An active credit or debit card is required on file for all patients. Your card will be charged 48 hours before your scheduled appointment. If payment is declined, you will be notified immediately and given a 24 hour window to update your payment information to retain your appointment time, otherwise your appointment will be cancelled.
Appointments booked within 24 hours of the appointment time will be charged in full at the time of booking.
HSA and FSA cards are accepted. I do not take checks or cash at this time.
Insurance
I do not participate with any insurance panels at this time.
I am considered an out-of-network provider, which means your insurance may reimburse a portion of your costs. A Superbill will be provided within 48 hours after every appointment and will contain everything your insurance company needs to process a reimbursement claim, including diagnosis codes, procedure codes, your date of service, and my NPI number. You submit the Superbill directly to your insurance company and reimbursement often takes about 1-2 months.

You may also be able to use visit costs as a tax-deductible out-of-pocket medical expense, please speak with your tax preparer or accountant about whether this applies to you. I am happy to provide the necessary documentation if this is something you decide to do.
Cancellation Policy
I require 48 hours notice for cancellations.
- Late cancellation (less than 48 hours notice): 50% of the scheduled appointment fee
- No-show (no contact at time of appointment): 100% of the scheduled appointment fee
- Patients with a pattern of late cancellations or no-shows will be required to pay in full at the time of booking in order to continue scheduling.
- Patients who continue a pattern of late cancellations or no-shows despite the prepayment requirement may be discharged from the practice at my discretion.
Cancellations due to acute illness, family emergency, or similar extenuating circumstances are always handled with compassion — please just reach out and let me know.
If you are more than 15 minutes late your appointment may need to be cancelled, in which case a 50% cancellation fee will apply.
Refunds
If you cancel within the 48-hour window, any applicable refunds will be processed within 24 hours of your original appointment time.
New Patient Paperwork
Before your first appointment you will receive intake forms through Charm, my secure patient portal. Please complete all paperwork and upload any prior labs, imaging reports, or medication lists at least 10 days before your appointment as this gives me adequate time to review everything before we meet. If paperwork arrives closer to your appointment date I will do my best to review it, but our time together may be more limited as a result.
Minors
Pediatric massage is available for children 12 and under. Patients ages 13-17 are welcome and will be scheduled at adult rates. A parent or guardian must provide written consent and be present in the room for all patients under 18.
Accessibility
Please contact me prior to your appointment if you have mobility concerns so we can discuss whether this space will meet your needs.
Parking
Parking is available on site.
Termination of Care
I reserve the right to discontinue care with appropriate notice if the therapeutic relationship is no longer working, if there is a pattern of policy violations, or if your needs are outside my scope of practice. In non-emergency situations I will provide 30 days notice and referrals to appropriate alternative care. Emergencies will be handled on a case by case basis with appropriate referrals provided.
